CPS HEALTH SCREENER has Changed

A new “HOME HEALTH QUESTIONNAIRE” for families has replaced the Health Screener.
Dear Holden Families,
CPS Health Screener has made changes to the Health Screener process that will make coming to school much easier for family members. The Wellness Screener will be replaced with the Wellness Set of questions and the Attestation kind. Please read the updated instructions below for more information so you will know what to do each morning before bringing your child to school. The success of this process depends on everyone staying safe.
At-Home Daily Health Questionnaire:
The staff and pupils will certainly complete a daily health and wellness checklist in the house before they come to school using the CPS Health Screener wellness testing concerns. The set of questions will not be submitted electronically. (A link to the personnel screener can be discovered affixed listed below).
Moms and dads and also guardians will certainly be called to submit.
A quarterly attestation form that they approve and complete before sending their trainee to college includes daily well-being checks. The attestation will certainly ask moms and dads as well as guardians to:
- Consent to screen their youngster for feasible signs and symptoms or COVID-19 direct exposure at home each day before school
- Verify that they will certainly not send their kid to school if they respond “yes” to any one of the CPS wellness testing questions
- Inform their kid’s college if they will certainly be lacking from in-person courses since they are sick or have been subjected to a verified COVID case.
By September 2, 2021, for the first quarter, we require all homes to authorize and submit the attestation forms to the school.
